New Patients

• Please ask your pediatrician or primary care doctor to write a referral and prescription for speech therapy, occupational therapy, or physical therapy. Fax referrals to 770 622 2534.
• Call the office at 770 622 2532 to schedule the initial evaluation.
• The office administrators will verify insurance (Amerigroup, Peachstate, Caresource, SSI) prior to evaluation.
• Parents will fill out patient care information prior to the evaluation.
• AIPT requests one caregiver to attend the evaluation to provide case history in the evaluation.
• The therapist writes the evaluation and plan of care to be sent to the primary care doctor.
• After approval from insurance, we are excited to start therapy for your child. We are open Monday to Friday 8 am to 7 pm.
